General Price List for Adult Services
Prices effective as of December 1, 2012 and are subject to change without notice.
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
Prior to drafting any contract for goods or services, the responsible party or the decedent's survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent.
PROFESSIONAL SERVICES AND FACILITIES
Basic Professional Service and Use of Facility Fee:
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forward or receiving remains.) Basic professional services, overhead and basic use of facilities. $2,350.00
Transfer of remains to funeral home:
Transfer to funeral home $395.00
Additional mileage charge one way beyond 35 mile radius of Jay Chapel $2.50
Professional Embalming:
Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement which does not require you to pay for it, such as direct cremation or immediate burial.

Visitation:

Funeral or Memorial Services:
Additional services of funeral director & staff, use of our facilities and equipment for funeral services. Planning the funeral or memorial service, placement of obituary notices, direction and supervision of the services.
Funeral service in our chapel or at a church and graveside services $950.00
Additional evening prayer service in our chapel or at a church $450.00
Funeral or memorial service in our chapel or at a church that concludes at chapel or church $795.00
Graveside service only $795.00
Overtime staff fee for Saturday, Sunday or holiday services $300.00

MEMORIAL TRIBUTE ON DVD PACKAGE: Includes scanning and set up of first 25 photos on DVD, use of projector and screen and one DVD $150.00

ALTERNATIVE SERVICES
Forward remains to another funeral home: Basic professional services and use of facilities, local transfer of remains, professional embalming, and dressing & casketing. (This charge does not include casket or container, visitation, funeral services, shipping or transportation costs.) $3,395.00
Receiving remains from another funeral home: Basic professional services, use of facilities and funeral coach to cemetery. (This does not include visitation, funeral or graveside service, or transportation from the airport.) $2,600.00
Forwarding Remains Outside the United States: Due to coordinating and filing the required paperwork with embassies and consulates, there is an additional service charge for this type of arrangement. This charge does include our staff and procurement vehicle. In some instances, there are extraordinary requirements where additional charges may be added. This charge does not include the consulate and/or other governmental required cash advance fees for the transfer. (This is an additional charge) $450.00
Immediate Burial: Basic professional services and use of facilities, local transfer of remains to funeral home, sanitary care, dressing & casketing, use of refrigerated holding facility (2 days), and funeral coach to cemetery. (This does not include visitation, sales tax on casket or container, or other options available.)
IMMEDIATE BURIALS RANGE IN PRICE FROM $3,545.00 to $17,240.00
Immediate burial, with casket selected at funeral home, the price of the casket & sales tax will be added to this fee $3,545.00
Immediate burial, with basic shaker pine composite wood casket and ivory crepe interior $4,190.00
Immediate burial, with casket or container provided by purchaser $3,545.00
Limited private family viewing (up to one hour) $100.00
Direct Cremation: Basic professional services and use of facilities, local transfer of remains to funeral home, sanitary care, use of refrigerated holding facility (2 days), and transportation to crematory in Fresno. (This fee does not include visitation, funeral or memorial service, crematory fee or urn.)
If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are fiberboard.
DIRECT CREMATION RANGE IN PRICE FROM $3,220.00 to $15,220.00
Direct cremation, with casket or alternative containers selected at funeral home, the cost of the casket with sales tax will be added to this fee. $3,220.00
Direct cremation with fiberboard alternative container, not including state sales tax $3,415.00
